No water when I lift the lever

Lever up, but little or no water

• The boiler is empty. The manual: fill it.
• A plastic piston has unscrewed from its rod on some early-2000s machines (owner).
• Scale blocking the dip tube that draws water into the group (owner).

The manual also lists coffee too fine, too much coffee or tamping too hard when no coffee comes.
